Duties Conduct comprehensive assessments of patients' physical conditions, including musculoskeletal, neurological, and geriatric evaluations. Develop and implement customized treatment plans utilizing evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology interventions, and pediatrics. Manage discharge planning processes to ensure seamless transition from hospital or clinic to home or community settings. Document patient progress accurately in electronic health records (EHR) systems adhering to clinical documentation standards. Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to optimize patient care strategies. Educate patients and their families on treatment routines, injury prevention, and home care practices to promote ongoing recovery. Assist in outpatient rehabilitation programs at clinics or outpatient centers, providing expert care for orthopedic, neurological, pediatric, and geriatric patients. Experience Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Demonstrated expertise in acute care settings with knowledge of physiology, anatomy, medical terminology, and treatment plan development. Familiarity with outpatient rehabilitation center operations and outpatient clinic experience is highly desirable. Prior work with geriatric populations and in-home community care enhances your ability to deliver comprehensive patient care. Experience managing electronic health records (EHR) or electronic medical records (EMR) systems with attention to detail in documentation standards. Knowledge of discharge planning procedures and working collaboratively with multidisciplinary teams to ensure continuity of care. Strong client communication skills to effectively explain treatment options and motivate patients throughout their recovery journey.
