Licensed Physical Therapist providing personalized, in-home and senior community care to older adults.
Responsibilities:
Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness. Provide one-on-one care in private residences and senior living settings, including virtual care when appropriate. Document patient progress using electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place.
Qualifications & Skills:
Valid PT license or eligibility in the practice state. Degree from accredited PT program, basic EMR and computer proficiency, current CPR certification. Ability to meet physical demands such as lifting, repositioning, and assisting patients, including driving to patient locations.
