Responsibilities include:

Perform comprehensive physical therapy evaluations Develop individualized treatment plans based on patient goals Improve strength, balance, mobility, endurance, and functional independence Provide gait training and fall prevention education Recommend and educate patients on the proper use of assistive devices Collaborate with physicians and the interdisciplinary care team Educate patients and caregivers on exercises and home safety Document evaluations, treatments, and patient progress in the electronic medical record (EMR) Help reduce hospitalizations and improve patient outcomes through skilled therapy interventions Qualifications Required Graduate of an accredited Physical Therapy program Current Tennessee Physical Therapist (PT) license Current driver's license and reliable transportation with automobile insurance Current CPR certification (or willingness to obtain upon hire) Strong communication, documentation, and clinical decision-making skills Ability to work independently in patients' homes while collaborating with an interdisciplinary team Commitment to maintaining patient privacy and HIPAA compliance Preferred Home Health experience Experience with Medicare documentation Why Home Health?
