Job Description

Addus Family Company seeks a compassionate Physical Therapist to deliver high-quality, home-based rehabilitative care for seniors and individuals with disabilities. In this role, you will assess functional status, develop and implement individualized treatment plans, and provide hands-on therapy to improve mobility, safety, and independence. You will collaborate closely with nurses, caregivers, and families to coordinate care, educate on home exercise programs, and monitor progress. Responsibilities Perform comprehensive physical therapy evaluations in the home setting Develop and implement individualized treatment plans and goals Provide therapeutic exercises, gait and balance training, and functional mobility interventions Educate patients and caregivers on home exercise programs, safety, and equipment use Collaborate with interdisciplinary team members to coordinate patient care Document assessments, treatments, and progress notes in accordance with regulatory and agency standards Monitor patient progress and adjust treatment plans as needed Recommend and train patients on assistive devices and adaptive equipment Communicate changes in patient condition to physicians and care team promptly Participate in quality improvement and continuing education activities Required Skills Physical therapy evaluation and treatment planning Knowledge of musculoskeletal and neuromuscular conditions Gait, balance, and transfer training Therapeutic exercise prescription Home health documentation (EMR proficiency) Patient and caregiver education Use of assistive devices and durable medical equipment Pain management techniques within PT scope Fall risk assessment and prevention Interdisciplinary care coordination
