Position Summary The Physical Therapist (PT) provides skilled therapy services to residents, including evaluation, development, and implementation of individualized treatment plans to improve mobility, function, and quality of life. Responsibilities Perform physical therapy evaluations and re-evaluations in accordance with professional standards and payer requirements. Develop and implement treatment plans tailored to resident goals and functional abilities. Supervise and direct Physical Therapist Assistants (PTAs) in the implementation of treatment plans, as applicable. Complete timely, accurate documentation in the medical record and therapy system. Collaborate with the interdisciplinary team on care planning, discharge recommendations, and equipment needs. Educate residents, families, and staff on therapy programs, safety, and use of assistive devices. Qualifications Bachelor's degree or higher in physical therapy from an accredited program. Current, unencumbered PT license in the state of Tennessee. Long-term care/skilled nursing experience preferred. Strong clinical, communication, and time-management skills.
