Setting:

Long-term care / nursing home environment Responsibilities Assess residents' mobility, balance, strength, and functional status; develop and implement individualized PT treatment plans. Administer therapeutic exercises, gait training, manual therapy, and assistive device training as appropriate. Monitor patient progress, document findings, and adjust treatment plans to achieve functional goals. Collaborate with nursing staff and the interdisciplinary team to coordinate care and ensure patient safety. Educate residents and families on home exercise programs, precautions, and progression of activities. Maintain accurate, compliant documentation and meet regulatory requirements for licensing and reimbursement. Requirements Active Physical Therapist license to practice in Texas (or as required by state licensure). Experience in geriatric or long-term care rehabilitation preferred. Ability to work in a fast-paced LTC environment and manage multiple patient needs. Strong communication skills and proficient in standard PT documentation and record-keeping. Willingness to travel for a 13-week assignment and to work the listed Day shift. Demonstrated commitment to patient-centered care and interdisciplinary collaboration.
