Reform Rehab | Pegasus Landing of Chisholm Trail is currently in need of a Physical Therapist (PT) and/or Occupational Therapist (OT). Below are the full details of the position:
Overview:
Reform Rehab is a growing mobile outpatient therapy company providing therapy services within senior living communities. We are looking for a Physical Therapist (PT) and/or Occupational Therapist (OT) to provide consistent therapy coverage at our Pegasus Landing of Chisholm Trail location. The ideal candidate is dependable, patient-focused, and comfortable working with an older adult population in an assisted living/senior living environment. This position will help provide additional clinical coverage throughout the week, allowing Reform Rehab to maintain consistent patient care while continuing to grow.
Key Responsibilities:
Provide skilled physical therapy and/or occupational therapy evaluations and treatments based on the therapist's discipline. Develop and implement individualized plans of care. Treat primarily older adults with a variety of mobility, strength, balance, gait, functional, orthopedic, and activities-of-daily-living limitations.
For PTs:
provide therapeutic exercise, neuromuscular re-education, gait training, balance training, functional mobility training, and other appropriate skilled interventions.
For OTs:
provide therapeutic activities, ADL training, functional training, neuromuscular re-education, and other appropriate skilled interventions. Complete timely and accurate documentation and maintain compliance with Medicare and applicable regulations. Communicate effectively with patients, families, facility staff, and the Reform Rehab team. Coordinate care with other therapy disciplines, including PTs, OTs, PTAs, and COTAs as applicable. Maintain a professional and positive presence within the senior living community.
Qualifications:
Current Texas Physical Therapist or Occupational Therapist license in good standing. Graduate of an accredited PT or OT program. CPR certification preferred. Experience working with older adults or in a senior living setting is a plus. Strong communication, organization, and documentation skills. Reliable and comfortable working independently. New graduates are welcome to apply.
Pay Range & Benefits:
$60 per completed patient visit. One-on-one treatment sessions with one-hour time slots per patient. Flexible scheduling with the opportunity to build a consistent caseload. Target Location:
Primary location: Pegasus Landing of Chisholm Trail Keene, Texas We would primarily target candidates who live within approximately 20-30 miles of the Chisholm Trail location , with preference given to candidates who have a reasonable commute and are looking for consistent local therapy hours.
