Summary As a Physical Therapist at Coker Rehab you will play a vital role in delivering high-quality therapeutic care to our diverse patient population. This position is essential for enhancing the quality of life for our patients while upholding our mission of excellent healthcare service. Responsibilities Conduct comprehensive assessments of patients' physical abilities and needs. Develop individualized treatment plans based on patient evaluations and goals. Implement therapeutic interventions to improve mobility, strength, and overall function. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families about therapeutic exercises and home care strategies. Collaborate with other healthcare professionals to ensure coordinated patient care. Maintain accurate documentation of patient progress using Kinnser software. Participate in discharge planning to ensure continuity of care. Qualifications Licensed Physical Therapist in the state of Texas. Experience in geriatrics preferred. Strong knowledge of orthopedic rehabilitation practices. Excellent communication skills for effective interaction with patients and team members. Ability to develop comprehensive treatment plans tailored to individual patient needs. Proficient understanding of medical terminology related to physical therapy. Experience with discharge planning processes is a plus.
