Available Help for . Full time Physical Therapist needed for home health care. Must have current PT license. Must be willing to visit with patients in their home. Must have driver's license. Duties will include but not limited to: Provide comprehensive physical therapy services to adult and geriatric patients in their homes, focusing on maximizing functional independence and quality of life. Assess patient needs, develop individualized treatment plans, and implement therapeutic interventions to address mobility, strength, balance, and pain management. Collaborate with interdisciplinary teams, including nurses, occupational therapists, and physicians, to ensure a holistic approach to patient care. Educate patients and families on exercise routines, safety strategies, and adaptive techniques for daily activities. Maintain accurate documentation of evaluations, progress notes, and treatment outcomes in accordance with regulatory standards. Participate in ongoing quality assurance and professional development activities to ensure excellence in service delivery. Pay will be depending on experience and discussed with the employer.
