Duties Conduct comprehensive assessments of patients' physical conditions to develop personalized treatment plans aligned with clinical goals. Implement evidence-based physiotherapy techniques such as trigger point therapy, manual therapy, and therapeutic exercises to promote healing and mobility. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and optimize patient outcomes. Manage discharge planning processes by evaluating patient progress and preparing appropriate care instructions for ongoing recovery at home or in community settings. Maintain accurate and timely documentation of treatment sessions in electronic health records (EHR) systems in accordance with clinical documentation standards. Educate patients and their families on treatment procedures, rehabilitation exercises, and safety measures to support long-term health improvements. Assist in developing and adjusting treatment plans based on patient progress, physiology knowledge, and evolving medical needs across various settings such as inpatient units or outpatient clinics. Qualifications Valid state licensure as a licensed Physical Therapist with practicing experience in a physical therapy role. Demonstrated experience working with geriatric patients, pediatrics, neurology cases, orthopedics, or outpatient rehabilitation centers. Strong understanding of anatomy, physiology, medical terminology, and clinical documentation standards necessary for effective patient care. Proven ability to develop treatment plans that incorporate discharge planning and patient rehabilitation strategies. Experience working in acute care settings such as hospitals or inpatient units is preferred; outpatient clinic experience is highly valued. Familiarity with EMR (Electronic Medical Records) systems and outpatient/outpatient rehabilitation documentation practices. Excellent client communication skills with the ability to collaborate seamlessly with healthcare teams across diverse environments including home health and community care settings.
