Job Description
Physical Therapist (PT) - Per Diem Goldthwaite Health & Rehab Center 1207 Reynolds Street, Goldthwaite, TX 76844 Join Our In-House Subacute Rehabilitation Team! Goldthwaite Health & Rehab Center is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our in-house subacute rehabilitation team. This is an excellent opportunity to make a meaningful impact by helping residents improve their mobility, strength, balance, functional independence, and overall quality of life while working alongside a dedicated interdisciplinary team. Responsibilities Evaluate residents' physical function, mobility, strength, balance, endurance, and functional limitations. Develop individualized treatment plans based on each resident's goals, medical condition, and clinical needs. Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, and functional independence. Assist residents with gait training, transfer training, therapeutic exercises, neuromuscular re-education, and functional mobility activities. Recommend and train residents in the appropriate use of assistive devices and adaptive equipment. Educate residents, families, and caregivers on therapeutic exercises, safety strategies, fall prevention, mobility techniques, and home recommendations. Collaborate with physicians, nursing staff, occupational therapy, speech therapy, and other rehabilitation team members to coordinate patient care. Monitor residents' progress and modify treatment plans as clinically appropriate. Maintain accurate, timely documentation in accordance with facility policies and regulatory guidelines. Participate in interdisciplinary team meetings, care planning, and discharge planning as appropriate. Qualifications Doctor of Physical Therapy (DPT) or Bachelor's/Master's degree in Physical Therapy from an accredited program. Current Texas Physical Therapist license in good standing. Experience in skilled nursing, subacute rehabilitation, geriatrics, or a similar setting is preferred but not required. Strong clinical reasoning, communication, organization, and time-management skills. Ability to work effectively as part of an interdisciplinary rehabilitation team.
