Job Description

Memorial Hermann Health System seeks a PRN Physical Therapist to deliver high-quality, evidence-based rehab care for acute patients in our Southeast facility. In this per diem role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, collaborate with an interdisciplinary care team, and educate patients and families to support safe discharge and optimal recovery. You'll document in the EMR, follow regulatory and safety standards, and contribute to a culture of innovation, inclusivity, and continuous professional growth. Responsibilities Perform comprehensive patient evaluations in an acute care setting Develop and implement individualized physical therapy treatment plans Provide therapeutic interventions to improve mobility, strength, and function Educate patients and families on exercises, precautions, and discharge planning Collaborate with physicians, nurses, and interdisciplinary team members Document assessments, treatments, and progress notes in the EMRAdhere to hospital, regulatory, and safety standards of care Assess patient readiness for discharge and make recommendations Participate in quality improvement and best practice initiatives Adjust treatment plans based on ongoing patient response and goals Required Skills Acute care physical therapy Patient evaluation and assessment Treatment planning and implementation Functional mobility training Post-surgical rehabilitation Documentation and EMR proficiency Interdisciplinary team collaboration Patient and family education Safe patient handling and transfer techniques Knowledge of rehab best practices and regulations
