Job Description

Memorial Hermann Health System is seeking a PRN Physical Therapist for acute care at our Southeast campus. In this flexible role, you will evaluate and treat diverse adult and geriatric patients with complex medical conditions, develop and implement evidence-based treatment plans, and collaborate closely with physicians, nurses, and rehab teams. You will educate patients and families, document care in the EMR, and ensure safe, compassionate discharge planning. Responsibilities Evaluate acute care patients and develop individualized treatment plans Provide evidence-based therapeutic interventions to improve mobility and function Collaborate with physicians, nurses, and interdisciplinary team members Educate patients and families on exercises, precautions, and discharge plans Document assessments, treatments, and progress in EMR accurately and timely Participate in discharge planning to ensure safe transitions of care Adhere to hospital, safety, and regulatory standards for patient care Contribute to a culture of innovation, inclusivity, and continuous improvement Required Skills Acute care physical therapy Patient evaluation and assessment Treatment planning and documentation Gait and transfer training Neuromuscular re-education Post-surgical rehab protocols Electronic Medical Records (EMR) documentation Interdisciplinary team collaboration Patient and family education Discharge planning
