Qualifications Driving Full Job Description Role:

Physical Therapist specializing in in-home, senior living, and virtual care for older adults.

Responsibilities:

Provide one-on-one therapy in various settings, including private residences and senior communities. Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments. Support patient wellness, mobility, and independence through therapy sessions. Document care accurately in electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging in place. Utilize telehealth to enhance care access and continuity.

Qualifications & Skills:

Valid PT license or eligibility in the practice state. Degree from an accredited program, with basic computer and EMR proficiency. Current CPR certification. Ability to meet physical demands such as lifting, repositioning patients, and driving.
