Job Description

Role Overview:

The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary environment. The role emphasizes accountability for program development, quality improvement, and productivity enhancement through flexible collaboration.

Qualifications & Skills:

Educational background in Physical Therapy (DPT preferred after 2015), with a requirement for a license as a Physical Therapist. Must hold a valid Texas Driver's License and Basic Life Support (BLS) certification. Relevant experience includes years of practice and specific types of experience, though specifics are preferred or required. Additional certifications such as Auto Liability Insurance and reliable transportation are required.
