Responsibilities Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans. Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs to promote recovery. Manage discharge planning processes by coordinating care transitions and providing patient education for ongoing self-care. Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to optimize patient outcomes. Maintain accurate clinical documentation adhering to medical terminology standards and electronic health records (EHR) management protocols. Work with adult geriatric patients, and those requiring home & community care. Monitor patient progress regularly and adjust treatment strategies to ensure optimal rehabilitation results in outpatient or inpatient settings. Qualifications Valid licensure or certification as a licensed Physical Therapist (PT) in the respective state. Proven experience practicing in a physical therapy role within skilled nursing facilities. Strong knowledge of anatomy, physiology, orthopedics, neurology, and geriatrics to inform effective treatment planning. Familiarity with clinical documentation standards and electronic health record (EHR/EMR) systems for accurate recordkeeping. Experience working with acute care settings and understanding procedures related to inpatient care in skilled nursing facilities. Ability to communicate clearly with adult patients while demonstrating empathy and professionalism. Knowledge of medical terminology relevant to physiotherapy practices such as trigger point therapy and rehabilitation techniques.
