You will collaborate closely with healthcare professionals, develop tailored treatment plans, and ensure seamless care transitions to promote optimal patient outcomes. Responsibilities Conduct comprehensive assessments of patients' physical condition, including mobility, strength, and functional abilities in home and community settings. Develop and implement individualized treatment plans that align with clinical best practices and patient goals. Deliver hands-on therapy interventions such as therapeutic exercises, manual therapy techniques, trigger point therapy, and gait training to facilitate recovery. Manage discharge planning processes by coordinating with healthcare teams to ensure smooth transitions from hospital or outpatient settings to home care. Maintain accurate and detailed clinical documentation adhering to standards for medical terminology, physiotherapy records, and electronic health records (EHR) systems. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient care strategies. Educate patients and their families on injury prevention, home exercise programs, and community resources to support ongoing rehabilitation and independence. Experience Proven experience practicing as a licensed physical therapist in outpatient clinics, inpatient hospitals, or home health environments. Strong background in outpatient rehabilitation center work with expertise in orthopedics, neurology, pediatrics, geriatrics, and geriatrics populations. Demonstrated ability to develop treatment plans based on physiology knowledge and anatomy principles tailored to individual patient needs. Familiarity with clinical documentation standards and medical terminology necessary for accurate record keeping within EMR (Electronic Medical Records) systems. Experience working with geriatric patients or those recovering from acute care episodes in inpatient or hospital settings is highly desirable. Knowledge of discharge planning processes and working with multidisciplinary teams to coordinate patient care effectively. Excellent client communication skills with a compassionate approach to patient care in home & community settings.
