Home Health Physical Therapist Location:
Fort Worth, TX Service Area Schedule:
Monday to Friday, 8:00 AM to 4:30
PM Weekend Requirement:
One weekend per month
Productivity Expectation:
30 points per week Position Overview We are seeking an experienced and compassionate Home Health Physical Therapist to join our team serving patients throughout the Fort Worth area. In this role, you will provide high-quality physical therapy services in patients' homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals. This is an excellent opportunity for a motivated clinician who thrives in an autonomous environment while delivering patient-centered care. Key Responsibilities Complete OASIS assessments in accordance with agency and regulatory requirements Conduct subsequent physical therapy visits and reassessments Develop and implement individualized treatment plans based on patient needs and goals Evaluate patient progress and modify treatment plans as appropriate Educate patients, families, and caregivers on exercises, safety, and disease management Collaborate with physicians, nurses, case managers, and other members of the healthcare team Maintain accurate and timely clinical documentation using Homecare Homebase (HCHB) Ensure compliance with all company policies, state regulations, and home health standards Required Qualifications Current Physical Therapist license in the State of Texas Minimum of one (1) year of home health experience Experience utilizing Homecare Homebase (HCHB) Strong clinical assessment and documentation skills Excellent communication and patient care skills Valid driver's license and reliable transportation Preferred Qualifications Candidates who stand out will have: Strong OASIS knowledge and documentation proficiency Excellent time management and organizational skills Ability to work independently while managing a daily caseload Experience serving diverse patient populations in the home health setting Additional Information Coverage area: Fort Worth service area Mileage reimbursement provided between patient visits (from first patient to last patient) Dress code: Navy scrubs
