Occupational Therapist (OT)

  • 4:30 PM No On-Call Requirement No Weekend Requirement Position Overview We are seeking an experienced Occupational Therapist (OT) to join our Home Health team.
In this role, you will provide in-home occupational therapy services, evaluating and treating patients in the comfort of their homes to help improve functional independence, safety, and quality of life. Key Responsibilities Conduct comprehensive occupational therapy evaluations in patients' homes. Develop and implement individualized treatment plans based on patient needs and goals. Provide skilled therapeutic interventions to improve daily living skills, mobility, strength, coordination, and overall function. Educate patients, families, and caregivers on treatment plans, home safety, adaptive equipment, and strategies to maximize independence. Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care. Complete accurate and timely documentation in Homecare Homebase (HCHB). Monitor patient progress and adjust treatment plans as needed. Ensure compliance with all clinical, regulatory, and agency standards.
QualificationsRequired:
Active Occupational Therapist (OT) License Home Health experience Experience using Homecare Homebase (HCHB) Strong clinical assessment and patient care skills Ability to work independently in a field-based environment Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
