We are seeking a motivated and passionate Physical Therapist (PT) to work on a per diem basis and make a difference in the lives of our residents. About the
Role:
As a Physical Therapist (PT) at Highland Care Center, you will have the opportunity to work with a diverse patient population and collaborate with our interdisciplinary team to develop and implement individualized care plans. Your expertise and compassion will enable you to empower our residents to achieve their maximum potential, promoting physical function, mobility, and overall well-being.
Responsibilities:
Conduct comprehensive patient evaluations and develop personalized treatment plans Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities Collaborate with nurses, occupational therapists, and other healthcare professionals to ensure interdisciplinary care Monitor patient progress, adjust treatment plans as needed, and communicate effectively with patients and families Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
Requirements:
Graduate degree in Physical Therapy from an accredited institution Current licensure as a Physical Therapist in the state of Utah Ability to work a variable schedule, including weekends and holidays, on a per diem basis
