What You'll Do:

Provide skilled physical therapy interventions and personalized exercises to patients in their homes. Develop individualized treatment plans tailored to each patient's needs. Monitor progress and update physicians, case managers, and families on patient outcomes. Document care accurately and efficiently in compliance with regulations. Promote patient safety and comfort through proper body mechanics and safety protocols. Maintain high ethical standards in line with the American Physical Therapy Association.

What We're Looking For:

Education:

Graduate of a PT program accredited by the APTA or the AMA's CMEH.

Skills:

Excellent communication, professionalism, and dedication to patient privacy.
