Job Description

A Plus Healthcare at Home is expanding into Park City and looking for a PRN Physical Therapist to help patients recover where they do it best: at home. Many of our Park City patients are coming home after orthopedic and other surgeries, so your work will have a direct, visible impact on getting people back to the active lives they love. Build a schedule that fits your life, whether you're supplementing a clinic or hospital position or want flexible hours around family. Position Highlights PRN / flexible schedule, you help set your availability Park City and surrounding areas
Pay:
negotiable based on experience $0.50/mile reimbursement One on one treatment time in the patient's home, without the pace of a busy clinic Supportive team environment Opportunity to grow into a full-time position as the territory grows What You'll Do Perform PT evaluations and reassessments in the home Develop and carry out individualized treatment plans in coordination with the physician and care team Provide therapeutic exercise, gait and transfer training, balance work, and post-surgical rehab Assess home safety and recommend equipment or modifications Teach patients and caregivers home exercise programs and fall prevention Communicate progress and changes to the physician and RN case manager Complete timely, accurate documentation Plan for discharge from the start of care We're Looking For Someone Who Holds a current Utah Physical Therapist license and CPR certification Has strong evaluation and clinical reasoning skills Is comfortable working independently in the home setting Has excellent communication and documentation skills Is reliable and patient focused Has a valid driver's license, reliable vehicle, and auto insurance Home health experience is preferred but not required
Physical Requirements:
Able to walk, stand, reach, bend, and kneel for extended periods, and to assist with patient transfers and mobility.
