Job Summary We are seeking a dedicated and energetic Home Health Physical Therapist to join our dynamic healthcare team. In this vital role, you will provide expert physical therapy services directly in patients' homes, promoting recovery, independence, and improved quality of life. Your expertise will help develop personalized treatment plans, facilitate patient rehabilitation, and collaborate effectively with a multidisciplinary team to ensure optimal patient outcomes. Duties Assess patients' physical conditions and develop tailored treatment plans to address their specific needs, including orthopedics, neurology, pediatrics, and geriatrics. Implement therapeutic interventions such as manual therapy, trigger point therapy, and mobility exercises to promote recovery and functional independence. Conduct discharge planning and coordinate care with healthcare professionals to ensure seamless transitions across inpatient, outpatient, and home health settings. Maintain accurate clinical documentation standards within electronic health records (EHR) systems, including progress notes, treatment summaries, and care plans. Collaborate with physicians, nurses, occupational therapists, and other team members to optimize patient care strategies. Educate patients and their families on physiotherapy techniques, injury prevention, and home exercise programs to support ongoing recovery. Monitor patient progress regularly and adjust treatment plans accordingly to maximize rehabilitation outcomes. Requirements Valid state licensure as a Physical Therapist with practicing in a physical therapy role recognized by licensing boards. Proven experience in home health or outpatient rehabilitation centers with a strong focus on patient care. Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective treatment planning and record keeping. Experience working with geriatric patients as well as pediatric populations; familiarity with outpatient clinic experience is highly desirable. Ability to perform discharge planning efficiently while collaborating with healthcare professionals across various disciplines. Proficiency in electronic health records (EHR) management systems such as EMR platforms used in outpatient or inpatient settings. Strong communication skills for client interaction and education; experience working with home & community care environments is preferred. Knowledge of outpatient rehabilitation techniques including trigger point therapy and orthopedic treatments; inpatient or hospital experience is advantageous. Pay:
$90.00 - $130.00 per hour Expected hours: 40.0 per week
