Job Summary Physical Therapist (PT) - Community Health Center | Contract Alexandria, VA | 12-Week Contract | $53/Hour Halcyon Nurse Staffing is seeking an experienced and compassionate Physical Therapist (PT) for an urgent 12-week contract assignment at a community health center in Alexandria, Virginia. This is an excellent opportunity for a qualified PT seeking a consistent weekday schedule with guaranteed hours.

In person Job Summary The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. The ideal candidate will have strong clinical skills, effective communication, and the ability to work collaboratively with a multidisciplinary healthcare team. Responsibilities Conduct physical therapy evaluations and assess patients' mobility, strength, balance, and functional limitations. Develop and implement individualized treatment plans based on patient needs and goals. Provide therapeutic exercises, mobility training, and rehabilitation interventions. Monitor patient progress and modify treatment plans as appropriate. Educate patients and families on exercises, mobility techniques, and injury prevention. Document evaluations, treatment sessions, progress notes, and discharge plans accurately. Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care. Promote patient safety and follow clinical guidelines, infection-control practices, and facility policies. Requirements & Qualifications Active Physical Therapist license in Virginia or eligibility to practice in Virginia. Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program. Current CPR/BLS certification (if required by the facility). Previous experience providing physical therapy in a clinical, outpatient, rehabilitation, or community healthcare setting preferred. Strong knowledge of physical therapy evaluations, treatment planning, and rehabilitation techniques. Ability to assess patient needs and provide individualized, evidence-based care. Excellent communication, interpersonal, and patient-education skills. Strong documentation, organization, and time-management abilities. Ability to work independently and collaboratively within a multidisciplinary team. Ability to commit to the full 12-week assignment and guaranteed Monday-Friday schedule. Must be able to complete required credentialing and onboarding before the start date.
