Duties Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, balance, and functional abilities. Develop individualized care plans aligned with clinical documentation standards and patient goals. Implement evidence-based treatment techniques such as therapeutic exercises, manual therapy, trigger point therapy, and gait training to facilitate optimal recovery. Coordinate discharge planning and collaborate with healthcare professionals across inpatient, outpatient, and community settings to ensure seamless patient transitions. Maintain accurate and detailed electronic health records (EHR) and medical documentation in compliance with regulatory standards. Educate patients and their families on injury prevention, home safety modifications, and ongoing self-care strategies. Monitor patient progress regularly, adjusting treatment plans as needed to maximize outcomes in areas such as orthopedics, neurology, pediatrics, or geriatrics. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice. Proven experience working with geriatric patients and familiarity with home & community care environments. Expertise in outpatient clinic settings or outpatient rehabilitation centers is highly desirable. Ability to develop effective treatment plans for diverse populations including pediatrics and neurology cases. Proficiency in managing electronic health records (EHR) systems and EMR systems for accurate documentation. Excellent client communication skills to educate patients on therapy exercises and safety protocols. Experience in discharge planning, acute care environments, inpatient hospital settings, or outpatient clinics enhances your ability to deliver comprehensive care.
