QUALIFICATIONS

: Licensed or registered to practice as Occupational Therapist / Physical Therapist in the state of Virginia Preferably at least two (2) year of experience as Home Health PT Valid Virginia Drivers License Medicare Certified Home Health Care Agency, HORIZONS

, is looking for Home Health Physical Therapist in Fairfax/Alexandria/ Arlington area . We are serving the healthcare needs of our community by providing services needed in order to allow an individual to remain safely and comfortably in their home. The PT/OT under physician orders, is responsible for the treatment/management of clients needing PT services in the home setting. The therapist will provide in-home, one-on-one care to a predominately geriatric population. This home health position will give providers the opportunity to apply their therapy skills, while allowing for a flexible schedule. Responsibilities include proficiency in PT /OT management/treatments. The ability to work independently required. Excellent communication and interpersonal skills essential. Records clinical/progress notes in the patient's clinical record weekly. Instructs the family in the client/patient's total speech therapy program and evaluates the home environment. Instructs other involved agency personnel in the care of client/patients receiving physical/occupational therapy. Plan for patient's discharge, prepares discharge instructions. Computer Skills for documentation and able to drive to Client's home.

• Provide therapy services consistent with the patient's Plan of Treatment.
• Observe patient during treatment, compile data and report patient's progress.
• Act as a liaison and maintain contact with patients, family and Clinical Team to ensure effective care coordination.
• Provide hands-on care and education of the patient Qualifications
• Degree from an accredited Physical / Occupational Therapy program
• Home Health experience a preferred
• Current and unrestricted VA Physical Therapy / Occupational Therapy License
