Responsibilities include performing comprehensive evaluations, developing individualized treatment plans, improving functional mobility, reducing pain, restoring independence, and collaborating with physicians and the interdisciplinary care team to achieve the best possible patient outcomes. Responsibilities Perform comprehensive physical therapy evaluations. Develop individualized treatment plans based on physician orders and patient goals. Provide therapeutic exercises, gait training, balance training, strengthening, transfer training, and neuromuscular re-education. Evaluate home safety and recommend environmental modifications to reduce fall risk. Assess the need for durable medical equipment (DME) and assistive devices. Educate patients and caregivers on home exercise programs, safety, disease management, and injury prevention. Monitor patient progress and adjust treatment plans as appropriate. Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals. Complete accurate and timely electronic documentation. Participate in interdisciplinary case conferences and quality improvement initiatives. Maintain compliance with Medicare, Medicaid, HIPAA, state regulations, and agency policies.
