Work Environment This position is based in our state-of-the-art Rehabilitation Center located in Portsmouth, Virginia, where you will work directly with patients in an onsite setting. The center is equipped with advanced therapeutic technologies and resources to provide an optimal recovery experience. Our team-oriented atmosphere encourages collaboration between healthcare professionals, guaranteeing comprehensive care for every patient. The working hours are set from Monday to Friday, offering a favorable work-life balance while committing to the highest standards of patient care. Key Responsibilities As a Physical Therapist at Forest Manor, your essential duties will include: Conducting thorough evaluations of patients by reviewing medical histories, performing relevant assessments, and developing personalized treatment plans based on findings. Communicating evaluation results and treatment goals effectively to patients and their families, ensuring they are active participants in their rehabilitation journey. Implementing treatment interventions that align with the best practices in physical therapy using evidence-based approaches. Monitoring and documenting progress regularly, adjusting the treatment plan as necessary to accommodate changes in patient conditions. Collaborating with multidisciplinary healthcare teams to ensure coordinated care and timely referrals for additional services when needed. Participating in discharge planning and follow-up care coordination to maximize patient outcomes post-therapy. Maintaining accurate and timely documentation in compliance with healthcare regulations, ensuring transparency and clarity in patient care records. Engaging in community outreach and educational initiatives to promote awareness and access to rehabilitation services. Contributing to the team by staying abreast of the latest rehabilitation techniques and participating in continuing education for professional growth. Patient Population / Client Focus Our facility treats a diverse population with varied needs, including individuals recovering from orthopedic injuries, surgeries, neurological conditions, and chronic pain. As a Physical Therapist, you will have the opportunity to work with a wide range of patients, tailoring your approach to accommodate their specific physical conditions and rehabilitation goals. You will engage with patients of all ages, providing compassionate care that recognizes their unique emotional and physical challenges during recovery. Ensuring each patient's comfort and motivation will be paramount as you guide them through their therapy sessions. Qualifications The ideal candidate will exhibit the following qualifications: A Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred. A valid state license to practice as a Physical Therapist in Virginia. Certification in Basic Life Support (BLS) from the American Heart Association. A commitment to providing patient-centered care with a strong emphasis on collaboration and improvement. Preferred Experience While prior work experience in a rehabilitation setting is not mandatory, candidates with a history of internships or clinical rotations in outpatient or inpatient physical therapy environments will be preferred. Experience with electronic health record systems for documentation will also be an asset.
