Key Responsibilities Conduct thorough evaluations of patients, including histories and physical assessments, to develop individualized treatment plans. Clearly communicate evaluation findings and treatment strategies to patients and their families, ensuring full participation in the goal-setting process. Implement targeted treatment protocols using evidence-based practices and adapt those protocols in response to patient progress and evolving healthcare standards. Collaborate effectively with a team of healthcare professionals to coordinate comprehensive patient care, making necessary modifications to treatment plans as conditions change. Document all aspects of patient care, including evaluations, treatment plans, progress notes, and discharge summaries, maintaining compliance with facility policies and regulatory standards. Create and manage discharge plans and follow-up care instructions to optimize patient recovery and ensure a smooth transition from therapy. Patient Population / Client Focus The ideal candidate will work with a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain. Qualifications A valid state license to practice as a Physical Therapist in Virginia. A Bachelor's degree in Physical Therapy or an equivalent degree from an accredited program. Ability to work collaboratively within a multidisciplinary team while demonstrating independence in clinical decision-making. Preferred Experience While not mandatory, prior experience in an outpatient rehabilitation setting is preferred.
