Work Environment This role operates primarily in a home care setting, enabling Physical Therapists to work directly with patients in the comfort of their own homes. Our approach recognizes the importance of personalized care and emphasizes flexibility and adaptability. As part of the home care team, you will collaborate with other healthcare professionals to ensure cohesive and comprehensive patient care while promoting our values of safety and professionalism. Key Responsibilities Conduct thorough patient examinations and evaluations to identify functional limitations and disabilities. Develop, implement, and adjust individualized treatment plans based on care assessments and patient progress. Provide expert consultation to healthcare providers regarding patient care and treatment strategies. Maintain accurate documentation of patient evaluations, treatment plans, and discharge processes in compliance with state regulations and professional standards. Educate patients and their families about treatment plans and rehabilitation goals to foster collaborative care outcomes. Engage in multidisciplinary communication with physicians, referral sources, and other team members to optimize patient care. Participate in community outreach and continuous quality improvement initiatives to enhance department operations. Supervise and mentor support staff, ensuring they have the necessary resources and training to assist in patient care. Attend regular staff meetings and professional development training to further personal competencies and enhance team collaboration. Patient Population / Client Focus Our patient population consists primarily of individuals experiencing orthopedic-related challenges, including but not limited to post-operative rehabilitation, sports injuries, age-related limitations, and chronic pain management. As a Physical Therapist at Strong Bodies, you will provide compassionate care while addressing each patient's specific needs, preferences, and recovery goals. Qualifications Bachelor's Degree in Physical Therapy from an accredited institution. Valid state license to practice as a Physical Therapist. Basic Life Support (BLS) certification. Solid written and verbal communication skills necessary for effective patient interaction and documentation. Proficient in word processing and basic computer skills to manage patient records and documentation efficiently. Preferred Experience While not required, candidates with prior experience in home care or outpatient settings will find a seamless transition into our practice. Familiarity with electronic health record (EHR) systems and a background in patient education techniques are also preferred.
