Job Description
Job Overview Sovereign Home Therapy Services LLC is a therapist owned home health contracting company operating in Virginia seeking a PT for the Stafford/Prince William/Fredericksburg area. We have contracts with multiple agencies in Northern Virginia and are expanding and in need of evaluating therapists to take on overflowing caseloads. Previous home health experience is a plus but is not required and training is offered for the electronic medical record system, home health rules and regulations, and 1099 operations. As a contractor you have complete control over your caseload and schedule and receive a much higher per visit rate blending flexibility and income while improving your patients mobility and quality of life! Duties Responsible for the evaluation, treatment, reassessment, and discharge of all referrals they accept. Promptly respond to referrals Write the plan of care for PTA to follow. Conference with PTA for patient care. Communicate with the home health agency to report status at evaluation, reassessment, discharge, and if any irregularities arise. Schedule their visits. Complete their invoice. Complete all documentation within 24 hours of visit
