Work Environment This role is predominantly onsite as part of our home care facility, providing physical therapy services directly in the homes of our patients. Key Responsibilities As an Orthopedic Physical Therapist at Bear Lake PT Clinic, you will: Evaluate, diagnose, and develop personalized treatment plans for patients facing functional limitations and disabilities due to orthopedic concerns. Utilize evidence-based practices to implement interventions, monitor progress, and adjust treatment strategies as necessary. Consult with other healthcare providers to ensure a cohesive approach to patient care, enhancing the continuum of care. Maintain accurate and timely documentation of evaluations, treatment plans, and patient progress, adhering to regulatory standards. Engage in patient education regarding injury prevention, physical health, and home exercise programs to promote recovery. Supervise and mentor support staff, ensuring high-quality delivery of patient care. Participate in community outreach initiatives and contribute to continuous quality improvement (CQI) projects. Patient Population / Client Focus Our patient population primarily includes individuals recovering from orthopedic surgeries, sports injuries, and those with chronic musculoskeletal conditions. We focus on providing personalized care tailored to each patient's needs, ensuring they receive the support necessary for effective rehabilitation. Qualifications The ideal candidate will possess the following qualifications: A Doctorate in Physical Therapy from an accredited institution. A valid state license to practice physical therapy. Basic Life Support (BLS) certification. Excellent written and verbal communication skills. Proficiency in documentation and electronic health record systems. Preferred Experience While all qualified candidates are encouraged to apply, we prefer candidates with: Experience in orthopedic physical therapy with strong knowledge of evidence-based treatment modalities. Previous experience in home health settings. Demonstrated ability to collaborate with interdisciplinary teams and engage in community-focused initiatives.
