Responsibilities:

Evaluate and develop individualized treatment plans for residents Deliver skilled physical therapy services in accordance with physician orders Collaborate with interdisciplinary team members to optimize care plans Monitor, document, and adjust treatments based on resident progress Educate residents and families on therapy goals and outcomes

Qualifications:

Doctorate or Master's degree in Physical Therapy from an accredited program Current Vermont Physical Therapy license (or eligibility) Strong clinical, communication, and documentation skills Passion for working with geriatric and post-acute populations

State license to practice physical therapy required. Two years' experience in physical therapy preferred. Ability to direct others necessary. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
