1 Description American Traveler is seeking a Physical Therapist for a home health assignment requiring at least 1 year of home healthcare PT experience and licensure in VT and NH. Details Home health setting covering more than 70 towns across VT and NH, including White River Junction VT, Lebanon NH, Bradford VT, and Windsor VT areas Full time, day shift schedule Average patient visit length is 45 minutes Work as part of an interdisciplinary care team while maintaining independence in patient visits Requirements BS in Physical Therapy required Current BLS certification required Minimum 1 year of experience as a PT in a home healthcare setting Must hold a PT license in VT and NH, or an unencumbered compact license - license required if applicable ASHA, CHT, or ACP training certifications preferred Additional Information Responsibilities include screening, evaluating, re-evaluating, and treating patients, developing and carrying out treatment plans, documenting evaluations and goals, and teaching patients and families about care rationale Communicate and coordinate treatment activities with the interdisciplinary care team Work/life balance with flexible scheduling
