Physical Therapist Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our team at the Rutland Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider in Rutland, Vermont. As a Physical Therapist with our organization, you will play a vital role in revolutionizing care delivery by providing high-quality physical therapy services to our patients and residents. About the Role As a Physical Therapist, you will be responsible for: Developing and implementing individualized treatment plans for patients and residents Conducting thorough evaluations and assessments to identify areas of improvement Providing hands-on therapy sessions, education, and training to patients and residents Collaborating with other healthcare professionals to ensure comprehensive care Maintaining accurate and detailed records of patient progress Participating in ongoing education and professional development to stay current with best practices Responsibilities Evaluate patients and residents to identify physical therapy needs and goals Develop and implement individualized treatment plans to achieve patient goals Provide hands-on therapy sessions, including but not limited to: Exercise programs Modalities (heat, cold, electrical stimulation, etc.) Manual therapy Education and training on daily living skills and self-care Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists Maintain accurate and detailed records of patient progress Participate in ongoing education and professional development to stay current with best practices Requirements Bachelor's or Master's degree in Physical Therapy from an accredited program Possession of a current PT license in the state of Vermont About Us The Rutland Center for Living and Rehabilitation is a premier residential, long-term, rehabilitation, and subacute care provider that is dedicated to revolutionizing care delivery.
