As a Physical Therapist, you will play a crucial role in assessing and developing personalized care plans to promote optimal functional independence and safety for patients. You will collaborate with a multidisciplinary team to ensure the highest quality of care and patient satisfaction. Actively monitor and adjust patient visit schedules to maximize productivity and meet patient needs Perform comprehensive assessments, including OASIS and PT, to evaluate psychosocial, educational, safety, and risk factors Develop and implement individualized care plans with specific goals and interventions Provide skilled physical therapy treatments and assess patient progress during each visit Coordinate patient care with other clinical team members, including physicians and nurses Supervise and direct clinical activities of PTAs, PT students, and home care aides Complete and coordinate required documentation to ensure accurate billing and compliance Participate in in-service training programs and promote rehabilitation services Maintain and enhance personal knowledge and standards in physical therapy Engage in marketing and promotion of rehab services Perform various patient-related health assessments and procedures Communicate changes in patient care schedules promptly Collaborate with QI staff to ensure accuracy of clinical notes and documentation Ensure timely completion of agency, state, and federal forms Coordinate safe patient discharges and transfers Participate in regular peer review audits Need 1 year experience in setting Apply today and let Capstone Health help you reach your full potential!
