This field-based position focuses on achieving the highest standards of home health care and fostering patient independence.

Schedule:

Monday Friday, 8:00 AM 5:00 PM Approximate caseload of 2532 points per week (57 patient visits per day, depending on needs)

Key Responsibilities:

Conduct thorough physical therapy evaluations, including OASIS assessments as needed Develop and implement patient-centered treatment plans to address mobility, strength, safety, and daily living skills Deliver interventions such as exercise therapy, neuromuscular re-education, gait training, transfer training, and fall prevention Continuously assess progress and adjust care plans, collaborating with physicians and other healthcare team members Educate patients and caregivers on exercise routines, safety strategies, and proper use of assistive devices Document care promptly and accurately using Home Care Home Base (HCHB) Attend case conferences, interdisciplinary meetings, and participate in ongoing education Ensure compliance with all agency policies, regulatory standards, and Medicare guidelines

Qualifications:

Active Physical Therapist (PT) license in good standing At least 1 year of recent home health PT experience (field experience strongly preferred) Proficiency with Home Care Home Base (HCHB) Required OASIS assessment experience Sound clinical decision-making and strong functional assessment skills Valid drivers license, reliable transportation, and auto insurance Excellent communication and organizational abilities; comfortable working independently in the field
