Responsibilities:

Provide physical therapy services to patients in a home health setting within the specified radius. Document patient progress and treatment plans accurately by 10am the following day. Collaborate with a multidisciplinary team to ensure comprehensive patient care. Adhere to productivity expectations of a minimum of 30 points per week.
