Full Job Description Role Overview:

The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility setting, focusing on long-term support and short-term rehabilitation for diverse patient populations with orthopedic, neurological, and cardiopulmonary needs.

Key Responsibilities:

Develop and implement personalized treatment plans based on physician referrals. Assess and modify goals to ensure measurable, functional progress. Supervise and evaluate the care provided by PT assistants, aides, and students. Maintain accurate documentation and utilize therapy software effectively. Collaborate with interdisciplinary teams and communicate with patients, families, and support staff. Adhere to safety, confidentiality, and professional standards, including compliance with laws and regulations. Participate in community health initiatives and demonstrate excellent customer service. Ensure proper body mechanics and safety during patient care activities.

Qualifications & Skills:

Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT). Current licensure in applicable state, maintained in good standing. Preferred one year of experience; new graduates are welcome. CPR certification required or obtained during orientation. Proficiency in Microsoft Office Suite and effective communication skills. Ability to make independent decisions, demonstrate professionalism, and work effectively in a team environment.
