ASAP About the Role As a Home Health Physical Therapist, you will administer physician-prescribed physical therapy to patients on an intermittent basis in their place of residence, in accordance with the plan of care and under the direction and supervision of the Director of Clinical Services/Clinical Manager. You will work to improve or minimize residual physical disabilities and return individuals to their optimum functional level within their capabilities. Key Responsibilities Conduct initial assessments, develop plans of care, and perform reassessments every 30 days Provide physician-prescribed physical therapy in patient homes Perform all skilled procedures as ordered by the physician Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change-in-condition documentation within Agency-defined timeframes Utilize appropriate ICD-10 codes for all documentation Prepare clinical and progress notes accurately and in a timely manner Consult with physicians regarding changes in treatment and write progress reports Instruct patients and family/significant others in home programs and activities of daily living Participate with all home care personnel in patient care planning and care conferences Assist physician with evaluating level of function and help develop/revise the plan of care Direct and supervise Physical Therapy Assistants and Home Health Aides as assigned Participate in in-service programs and Quality Assessment and Performance Improvement activities Requirements Graduate of a PT program approved by CAPTE, APTA, or equivalent accrediting body — required Current Washington State PT license in good standing — required Minimum 2 years of experience as a Physical Therapist — preferred Home health or field-based clinical experience — a strong plus OASIS documentation experience preferred Ability to exercise initiative and independent judgment Valid driver's license and reliable transportation — required Strong communication, organization, and time management skills Why Apply Through Us?
