Job Summary We are seeking a dedicated and experienced Physical Therapist to join our healthcare team. The successful candidate will be responsible for providing comprehensive physical therapy services, primarily within a home health setting. This role involves assessing patients, developing treatment plans, and documenting clinical progress in accordance with industry standards. The ideal candidate will have at least one year of experience in home health care and be proficient in completing OASIS assessments and maintaining accurate clinical documentation. Duties Deliver individualized physical therapy treatments to patients in their homes, focusing on restoring mobility, reducing pain, and improving overall function. Conduct thorough home health assessments utilizing OASIS (Outcome and Assessment Information Set) to evaluate patient needs and develop appropriate care plans. Complete detailed clinical documentation in compliance with healthcare standards, ensuring accuracy and completeness of patient records. Develop and implement personalized treatment plans based on patient evaluations, goals, and progress. Collaborate with other healthcare professionals to coordinate patient care and optimize outcomes. Monitor patient progress regularly, adjusting treatment strategies as necessary to achieve desired results. Educate patients and their families on exercises, safety measures, and ongoing care requirements to promote independence. Maintain compliance with all regulatory guidelines related to home health services and documentation standards. Skills Proven experience in physical therapy, particularly within a home health environment (minimum of one year preferred). Strong knowledge of physical therapy techniques, assessments, and treatment modalities. Proficiency in completing home health assessments using OASIS tools. Excellent clinical documentation skills with an understanding of industry standards for accuracy and compliance. Ability to develop effective treatment plans tailored to individual patient needs. Strong communication skills for interacting with patients, families, and healthcare team members. Detail-oriented with excellent organizational skills to manage multiple patient cases efficiently. Knowledge of safety protocols and best practices in a home health setting. Pay:

$50.00 - $70.00 per hour Expected hours: 20.0 - 40.0 per week
