Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where you will play a vital role in restoring mobility, reducing pain, and improving the quality of life for diverse patient populations. In this engaging role, you will assess individual needs, develop personalized treatment plans, and collaborate closely with healthcare professionals to deliver exceptional patient care. Your expertise in physiotherapy and physiology will be instrumental in guiding patients through their rehabilitation journeys across inpatient, outpatient, home health, and community settings. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans aligned with clinical standards. Implement evidence-based physiotherapy techniques, including trigger point therapy, orthopedics, neurology, pediatrics, and geriatrics to facilitate optimal recovery. Manage discharge planning and coordinate care with multidisciplinary teams to ensure seamless patient transitions across care settings. Document all patient interactions accurately using electronic health records (EHR) and electronic medical records (EMR) systems, adhering to clinical documentation standards. Educate patients and their families on treatment procedures, home exercises, and community resources to promote ongoing rehabilitation outside of clinical settings. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to refine treatment strategies and optimize patient outcomes. Stay current with advancements in physiotherapy practices and maintain compliance with medical terminology standards and clinical protocols. Experience Proven experience practicing as a licensed Physical Therapist in various environments such as inpatient hospitals, outpatient clinics, rehabilitation centers, or home health settings. Demonstrated expertise in acute care and outpatient rehabilitation with a strong understanding of physiology, anatomy, and medical terminology. Experience working with diverse patient populations including geriatrics, pediatrics, orthopedics, neurology patients, and those requiring discharge planning or home & community care. Familiarity with outpatient experience and outpatient clinic operations is highly desirable. Knowledge of clinical documentation standards for physical therapy documentation and proficiency managing EMR/EHR systems is essential. Prior involvement in treatment plan development focusing on patient rehabilitation goals while ensuring compliance with healthcare regulations. Ability to communicate effectively with clients of varying ages and backgrounds to foster trust and promote active participation in their recovery process.
