Job Description

Life Care Centers is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate, plan, and implement individualized treatment programs for senior residents, focusing on mobility, function, and quality of life. Collaborate closely with an interdisciplinary team to deliver patient-first, compassionate care in a family-focused environment. Responsibilities Evaluate patients using standardized assessments to determine functional status, mobility, strength, balance, and pain levels. Develop and implement individualized, evidence-based treatment plans focused on restoring function and improving quality of life for primarily senior patients. Provide therapeutic exercise, gait and balance training, transfer training, and manual therapy as appropriate to patient needs and goals. Use therapeutic modalities safely and effectively, such as electrical stimulation, ultrasound, and heat/cold, in accordance with facility policies. Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and timely within the electronic medical record system. Educate patients, families, and caregivers on diagnosis, treatment plans, safe mobility strategies, and individualized home exercise programs. Collaborate with an interdisciplinary care team, including nurses, physicians, occupational and speech therapists, and social workers, to coordinate comprehensive care. Participate in care plan meetings, goal setting, and discharge planning to support safe transitions to home or other care settings. Monitor and report changes in patient condition, treatment response, or safety concerns to the appropriate clinical team members. Adhere to all professional, ethical, regulatory, and facility standards, including infection control, safety procedures, and documentation requirements. Contribute to quality improvement initiatives and outcome tracking within the facility to support excellent healthcare results. Support a patient-first, family-focused culture by demonstrating empathy, clear communication, and respect for residents' choices and dignity.
