Implement therapeutic exercises, manual therapy, and modalities to promote healing and restore movement. Monitor patient progress regularly, adjusting care plans as needed to optimize outcomes. Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care strategies. Provide discharge planning and education to patients and their families for continued care at home or in community settings. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital units, or home health environments. Demonstrated expertise in developing treatment plans for diverse populations including geriatrics, pediatrics, orthopedics, and post-surgical rehabilitation. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for accurate patient care records. Prior involvement in outpatient rehabilitation centers or outpatient clinic settings is preferred.
