Job Description
Job Overview We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will deliver expert rehabilitative care to diverse patient populations, including those in outpatient clinics, hospitals, inpatient units, and home health settings. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through personalized treatment plans. Responsibilities Assess patients' physical conditions by reviewing medical histories, conducting examinations, and evaluating mobility and functional abilities. Develop comprehensive treatment plans tailored to individual needs, incorporating physiotherapy techniques such as trigger point therapy and orthopedics interventions. Implement therapeutic exercises, manual therapy, and modalities to promote healing and restore movement. Monitor patient progress regularly, adjusting care plans as needed to optimize outcomes. Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care strategies. Provide discharge planning and education to patients and their families for continued care at home or in community settings. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital units, or home health environments. Demonstrated expertise in developing treatment plans for diverse populations including geriatrics, pediatrics, orthopedics, and post-surgical rehabilitation. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for accurate patient care records. Prior involvement in outpatient rehabilitation centers or outpatient clinic settings is preferred.
