Role:

Licensed Physical Therapist providing personalized in-home, senior living, and virtual care to older adults.

Responsibilities:

Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document care using EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.

Qualifications & Skills:

Valid PT license, degree from an accredited program, proficiency in EMR and computers, current CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.

Support patient wellness, promote aging in place, and support safety through education. Flexibility in schedule creation, working in various settings, and utilizing virtual care options are emphasized.
