Edenbrook Sheboygan a skilled nursing facility in the Eden Senior Care Network, is now hiring a Physical Therapist to provide rehabilitation services in one or more of our service locations!
Position:
Full-Time Physical Therapist (PT)
Primary Job Responsibilities:
Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance Identify need for and make referral to other disciplines to address the comprehensive needs of the patient Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards Develop care plans using a variety of treatment techniques Create fitness- and wellness-oriented programs tailored to patients' specific needs Provide quality, personalized and evidence-based care and proven interventions Motivate patients during treatment in order to help them function optimally Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination Evaluate effects and monitor and communicate progress
Qualifications:
Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program. Active Wisconsin Physical Therapy (PT) license in good standing. Experience in skilled nursing, home health, or rehabilitation settings preferred. Reasons to join
