Schedule:

Full-time, 13-week contract Monday to Friday, 8:00 AM 5:00

PM Caseload:

2532 points per week (approximately 57 patient visits per day) Daily travel within assigned territory (mileage reimbursement provided)

Core Responsibilities:

Complete comprehensive initial and ongoing physical therapy assessments, including OASIS documentation as appropriate. Develop and execute individualized care plans to improve mobility, strength, balance, and functional independence.

Deliver skilled interventions:

therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies. Evaluate progress, adjust plans of care, and communicate with physicians and the interdisciplinary team. Provide patient and caregiver education on exercises, adaptive equipment, safety, and home exercise programs. Accurately document all care in the electronic health record system (Home Care Home Base). Collaborate with nurses, occupational and speech therapists, social workers, and aides for coordinated, holistic care. Participate in team meetings and maintain regulatory and policy compliance.

Qualifications:
