Pride Health is hiring a Physical Therapist (PT) to support our client's healthcare facility in Lewisburg, West Virginia.

Pay Range:

$52.00 to $56.00/hour

•Pay offered is based on experience, expertise, credentialing, and education.

Duties:

Provide skilled physical therapy services to patients in a home health setting.

Perform comprehensive evaluations and develop individualized treatment plans to improve mobility and functional independence.

Educate patients and caregivers on therapeutic exercises, fall prevention, safety techniques, and home exercise programs.

Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate and optimize patient care.

Complete accurate and timely clinical documentation while meeting productivity expectations of 30 points per week.

Requirements:

Education:

Bachelor's Degree in Physical Therapy or higher from an accredited program

License(s): Active Physical Therapist (PT) License

Years of experience: Home Health experience preferred
