In this role, you will work closely with licensed physical therapists to provide high-quality patient care in the home health environment. Your primary focus will be to assist patients in their rehabilitation journey, helping them regain mobility and improve their overall quality of life. Duties Assist physical therapists in implementing treatment plans for patients with various conditions, including ortho, neuro and geriatrics. Provide direct patient care by guiding patients through exercises and therapeutic activities. Monitor patient progress and report any changes or concerns to the supervising physical therapist and/or Director. Educate patients and their families on treatment plans, exercises, and home care strategies. Maintain accurate documentation of patient treatments and progress in compliance with medical standards. Ensure a safe and clean environment for both patients and staff within the clinic or facility. Qualifications Associate's degree in Physical Therapy Assisting or a related field. Current state licensure or certification as a Physical Therapist Assistant. Knowledge of anatomy and medical terminology relevant to physical therapy practices. Experience in home health preferred. Strong interpersonal skills to effectively communicate with patients, families, and healthcare team members. Ability to work collaboratively within a team environment while demonstrating initiative in patient care.
