Job Description

South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, one-on-one therapy in post-acute and long-term care settings. In this clinician-centered, collaborative environment, you'll implement treatment plans under the direction of a Physical Therapist, focusing on restoring function, mobility, and independence for residents. Responsibilities include therapeutic exercises, gait and balance training, patient and family education, documentation, and participation in interdisciplinary care planning. Responsibilities Provide therapeutic exercises, gait, balance, and functional mobility training under PT supervision Implement treatment plans to improve strength, range of motion, and independence in ADLs Assist with transfer training, wheelchair mobility, and fall-prevention strategies Accurately document treatments, progress notes, and outcomes in the medical record Educate patients, families, and caregivers on exercises, safety, and home programs Collaborate with the interdisciplinary team to support resident-centered care plans Monitor patient response to treatment and report changes to supervising Physical Therapist Maintain compliance with state, federal, and facility regulations and ethical standards Contribute to discharge planning and recommendations for equipment or continued services Participate in ongoing education, in-services, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Skilled nursing/post-acute rehab experience Patient and family education Clinical documentation and EMR use Knowledge of rehabilitation modalities Fall risk assessment and prevention Interdisciplinary team collaboration Understanding of Medicare and rehab regulations
